Arrange a Cremation
To arrange a cremation with City Cremation, simply launch the Arrangement
Assistant by clicking the button below. You will be asked a series
of questions required by the State of California to complete a death
certificate. There may be information you do not know. Use the word
"Unknown" in those particular fields. Please note–
the word "Unknown" will appear on the death certificate
where you have indicated. No field can be left blank. You will NOT
be contacted for clarification of this information– the word
"Unknown" will be used in all fields left blank by you.
Once the information has been gathered, the Arrangement Assistant
will print the required forms, completed with your answers, along
with further instructions. Sign, initial, and date the forms where
indicated. Then fax the completed paperwork, along with any other
required documentation, to City Cremation as directed on the fax cover
sheet that will print with your forms. Once everything is in order,
we will procceed with the completion of arrangements, and will ship
you the ashes in 10 working days, or longer if death certificates
